At Illuminate Family & Cosmetic Dentistry, we want all our patients to have access to high-quality dental care when they need it and that is why we have designed three In-Office Dental Membership Plans.

Our Illuminate Dental Membership Plans provide the perfect solution to receive ALL of your routine preventative care services for one low annual membership* while saving money on other needed dental care. 

Too many people put off much needed treatment because of the cost. Along with your cleanings and exams, you will also receive a 20% discount on all additional dental services. Our Dental Membership Plans let you avoid the hassle and headache of dealing with traditional dental insurance while saving you money! 

What else should I know?

• Our membership plan is not insurance and is not a qualified health plan under the Affordable Care Act.
• Payment for any treatment(s) not included in the membership plan is due at the time of service.
• The membership plan benefits may not be combined with insurance, other offers, discounts or other discount plans.
• Plan discounts do not apply to cosmetic treatments such as cosmetic bonding and veneers orproducts available in our office.
• Some exclusions may apply.

*There is a one-year commitment to the Illuminate Membership Plan and if membership is cancelled prior to the end of the first year the participant will be responsible for the remaining balance. Membership plans may be cancelled at any time after the one-year commitment period is over. The credit card on file with your plan will automatically be charged annually.Your membership is non-transferrable and no refunds will be issued should you choose not to or be unable to use your membership for any reason.
 
The Illuminate Dental Membership Plan is only valid for services rendered at our office and cannot be used anywhere else.
